Complete this form and provide it to teachers, your school counselor, and/or other adults completing a college recommendation for you.
Early Graduation/Completion Request Form
Please see your School Counselor for this form, which is for students who wish to complete graduation requirements early and should be submitted during the 2nd semester of the sophomore year if the student aims to graduate at the conclusion of what traditionally would be the 11th grade year, or at the end of the 1st semester of the junior year if the student intends to complete all graduation requirements by December of the senior year.
Students with a Disability Requesting Accommodations
If you currently have an IEP or 504 Plan and use accommodations for both standardized testing and classroom testing, you may qualify for accommodations on the PSAT, SAT, or AP exams. To submit the request, counselors must have this form submitted and signed by the students and parent/guardian.

Teacher Forms
Grade Change Request Form - Once grades are stored, this form is documentation of a change in grade and is placed in the students permanent record. It must be signed by the teacher and administration prior to submitting the form to the School Counseling Office.
Parent and Graduated Student Forms
Parent Request to View Cumulative Folder - Parents are entitled to review the cumulative records for their children. This completed form will be placed in the student's file to document who has reviewed the folder and note any copies of information made.
